Standard
First User:                        $2,500 per year
Two or more Users:     $1,500/each, per year

Current promotional discount*
First User:                        $1,500 per year
Two or more Users:     $1000/each, per year

* If you subscribe during this discount period, the special pricing will be in effect for your first two years of use.  Year 3 and beyond will be billed at standard rates.  Subscriptions are billed in advance and are renewed annually base on the contract date.  Renewals will be billed automatically unless otherwise notified.

How to Subscribe:

1. Print, sign and date two (2) copies of the contract PDF. 
2. Print and complete the ACH Billing and User ID Request PDF.
3. Return the two original contracts and the completed ACH Billing and User ID Request form to either address, depending on the mailing service you prefer to use.
4. A fully executed original contract will be returned to you.
5. If enclosing a check, please make your check payable to CU Cooperative Systems, Inc.

This is the ACH Billing Authorization and User ID Request form.  It provides information about how you would like to be billed and the information needed to establish your account, user IDs and passwords.  An original copy must be completed and returned along with the contracts.
